CWC is available across the continental US for Blood panel, GLP-1, and hormone therapy. CWC requires members to have an active US address and phone number.
At this time, lab visits are not possible in Rhode Island, Hawaii, and Puerto Rico.

Blood Panel
You will be tested at one of over 2,000 Quest Diagnostics or Lab Corp locations across the US. Both Quest and Lab Corp perform billions of tests each year and are widely trusted by the medical community. Lab visits are not currently available in Rhode Island, Hawaii, or Puerto Rico.
It’s easy. After signing up with CWC, you will receive an email and text message to schedule a convenient time and location for your lab visit. You will be tested at one of over 2,000 locations nationwide, within 10 miles of nearly every home in the United States.
An average lab visit takes 15 to 30 minutes, as long as you are on time. Your first set of tests each year may require two lab visits to complete all tests.

Yes, but only for specific tests (included test here).
It is required that you fast for 8 hours before most tests. It is also strongly recommended to be well-hydrated to ensure easier blood draws and the most accurate urinalysis. If you do not fast, some of your biomarkers will be skewed. We also recommend a morning test for the most accurate results related to hormones such as testosterone and cortisol.
It is recommended that you do not discontinue any medication without discussing it with a doctor first.
Biotin (Vitamin B7) may affect results for the following tests: Testosterone, Cortisol, Thyroid Stimulating Hormone, T3, T4, Follicle Stimulating Hormone, Estradiol, and Luteinizing Hormone. Some choose to stop taking biotin 72 hours prior to testing.
Licensed clinicians both authorize your tests and review every result. A clinician's review is provided to you, along with your results.
Absolutely. CWC provides essential information to understand your results. A clinician reviews your complete results and provides notes that are an additional layer of context.
Weight Loss
Collaborative Wellness Center (CWC) provides personalized weight loss plans tailored to help you achieve your unique health and fitness goals. Our expert providers may recommend a holistic approach, combining GLP-1 injections with a custom diet plan crafted by our licensed nutritionist for optimal results.

You’ll begin by answering questions about your medical history and weight loss goals to create a custom weight loss profile. A licensed healthcare professional will review your information—all 100% online.
If treatment is right for you, you’ll receive a message from a provider with information about your personalized plan.
For people who haven’t had success losing body weight with lifestyle changes, counting calories, or over-the-counter options like supplements, weight loss medication can be a helpful addition to a comprehensive weight loss plan.
Be sure to share any underlying health conditions, health problems, or medical conditions with your healthcare provider as you start your weight management journey such as heart disease, type 2 diabetes, high cholesterol, high blood pressure, glaucoma, pancreatitis, thyroid conditions, blood sugar issues, constipation, heart disease, recent hospitalization, or any other adverse events in your health, or if you are pregnant or breastfeeding. Only take medical advice from a licensed medical provider.

No video visit is required; a provider will review your information 100% online. If prescribed, you’ll get unlimited online access to message your provider as needed for follow-ups, adjustments, and answers to your questions.
Unfortunately, GLP-1s are not yet available in all 50 states; however, CWC is working to expand our offering as soon as possible.
The ingredients are sourced from FDA-regulated suppliers and the treatments are compounded in FDA-regulated facilities.
Privacy and Security
Yes. CWC helps unlock, translate and organize your biological data. We think that information about your health is the most valuable data in the world to you. This is entirely your data and nobody else’s, including ours. You can opt to delete your data anytime you wish. Please know that by deleting it, we will not be able to recover it should you change your mind.
You, the laboratory, clinicians, and administrators. Clinicians and laboratories must report specific test results to state health authorities for disease tracking and prevalence.
CWC will not disclose your name, contact information, or any other personally-identifying information to any third-party advertisers. To be clear, everything we contact you about is specifically for your benefit.
Referrals
Yes. We’d rather invest in your health than traditional advertising.
We’re rolling out access to CWC Referrals gradually and will make it available to all members soon. Keep an eye on your email for your invitation—we’ll let you know as soon as you have access.
Once you’ve joined and signed up to receive payments with our Referral payment provider Impact. Function will provide you with a unique link that you can share.
You can earn One Hundred U.S. Dollars ($100.00 U.S.D.) Referral Fee for every new CWC member who (a) signs up for membership via your unique link, and (b) remains a CWC member for more than sixty (60) consecutive days. Payment for qualifying referrals occurs on a quarterly basis.
A Referral Fee is only paid for new, qualifying CWC members. We do not pay a Referral Fee for renewals or to existing members at this time.
You’ll receive payment for your Referral Fee(s) via direct deposit, to the account information you give to our referral program payment provider Impact.
No, any eligible Function member can participate in Referrals. Just share your unique referral link. You can refer your friends, your parents, your partner, your uncle—anyone who might be interested in taking control of their health.
Any active CWC member who abides by our Terms of Service - and who is not a licensed healthcare practitioner - can join Referrals. If your status changes in a way that affects your eligibility (e.g., becoming a licensed healthcare practitioner), notify us immediately by emailing referrals@collaborativewellnesscenter.com.
